MEDICAL RECORDS

At Westside Pediatrics we are committed to the protection and privacy of your family's medical information. We maintain a secure electronic medical record system and internal database server . We do NOT utilize, store, or reproduce your personal medical information on an external web based database or provide your information to other third party companies.

Our web portal system allows clients to access limited personal information. This online portal allows clients to set up a username and password.

Sensitive medical information cannot be accessed on the portal system. Parents and patients may download and print vaccination records, school forms, camp forms, communicate with the staff, and make appointments. Parents may opt out of the portal service at any time.

 

HIPAA

The following explains how your medical information can be appropriately used, disclosed, and accessed in accordance with The Privacy Regulations created as a result of the Health Insurance Portability and Accountability Act of 1996 (HIPAA).

 

We may disclose medical information to:

  • Health care providers for purposes related to your treatment

  • Obtain payment for services provided

  • Operate this medical practice

  • Contact and remind our patients about appointments

  • Patient's friends or family members that are involved in your child's care

  • Law enforcement officials; when required by federal, state or local law; or by a court/administrative order

  • Reduce or prevent a serious threat to your health and safety or the health and safety of another individual or the public. Under these circumstances, we will only make disclosures to a person or organization able to help prevent the threat.

  • U.S. or foreign military forces if you are a member and if required by the appropriate authorities

  • Federal officials for intelligence and national security activities authorized by law

  • Correctional institutions or law enforcement officials if you are an inmate or under Custody of a law enforcement official

  • Workers compensation and similar programs

 

You have the right to:

  • Request we communicate your child's health and related issues in a particular manner or at a certain location (home versus work)

  • Request we restrict our use or disclosure of your child's health information for treatment, payment, or health care operations. Additionally, you may request that we Restrict our use or disclosure to particular individuals involved in your child's care. We are not required to agree to these requests, but if we agree, we are bound by our agreement except when required by law, in emergencies, or when information is necessary to treat your child.

  • Obtain a copy of the information that may be used to make decisions about your child, including medical/billing records, but not including psychotherapy notes. Please direct this request to Westside Pediatrics at 310.979.7337

  • Amend our medical records if you feel they are incorrect or incomplete, as long as the information is kept by or for our practice. Please submit amendment requests and reasons that support your request in writing to Westside Pediatrics at 12301 Wilshire Blvd., Suite 120, Brentwood, CA 90025.

  • File a complaint with our practice or with the Secretary of the Department of Health and Human Services if you feel your privacy has been
